Arrived 11:30am Departed 1:10 pm An unannounced licensing inspection was completed today. Five children were in care for a total of 12 points. The children were playing in the play room in centers, eating lunch and resting. Eight children's records and two adult records were reviewed. An exit interview was completed at the conclusion of the inspection. Thank you for your cooperation. Please contact me if I may be of service.

Standard #: 22VAC40-111-100-A
Description: Based on the review of eight children's files the facility did not obtain physical health exams for all children in care. Evidence: 1. There are no physicals on file for child #2 and 3. Enrollment date 8/26/15

Plan of Correction: The provider has requested the physicals from the parent and will request them again.

Standard #: 22VAC40-111-270-B
Description: Based on observation the facility did not maintain locked ammunition. Evidence: 1. During a tour of the firearm storage area, the LI observed that the ammunition drawer was not locked.

Plan of Correction: The provider will place the ammunition in a locked ammunition locker.

Standard #: 22VAC40-111-320-E
Description: Based on observation the facility did not secure a space heater. Evidence: 1. The LI observed a space heater on top of a wooden shelf. 2. The heater was in use and was hot to the touch.

Plan of Correction: The heater was turned off during the inspection. It will not be used with children in care.

Standard #: 22VAC40-111-680-A
Description: Based on observation the provider did not wash her hands after each diaper change. Evidence: 1. The provider changed two diapers in a row and did not wash her hands between the diaper changes.

Plan of Correction: The provider will wash her hands between each diaper change.
