Comments:
An unannounced monitoring inspection was done 10/19/2105 beginning at 9:30 a.m. and ending at 11:30 p.m. The licensee and household member was present with the licensee's infant. The points were at four and in compliance. The LI reviewed three children's and on staff and a household member's record during the inspection. Discussion about upcoming changes to the licensee's household, and future plans.

Standard #: 22VAC40-111-780-A
Description: Based on review of three records, the licensee failed to ensure that all emergency information. Evidence: 1. The inspector found that child # 1 did not have documentation of an annual update for the child's emergency information.

Plan of Correction: The licensee will have the parent to review all information in the record and will monitor all records.
