Technical Assistance:
The licensing inspector informed the provider that a household member will be 14 years of age prior to the next visit, and will require the Central Registry Background check to be completed.

An unannounced renewal inspection was conducted on 10/01/2015. It began at 9:30am and concluded at 11:00am. There were 3 children (6 points) in the direct care of the provider. Children were observed napping, playing with age appropriate toys, and sharing. Children were also observed following proper hand washing procedures. Violations were observed and discussed with the provider. A sampling of children's records, the provider's record, household records, the physical space, the care of children, and evacuation drills were inspected during this visit. Standard #: 22VAC40-111-100-A
Description: Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Child #2 has been in care, per provider interview, for over a year and there is not documentation of a physical conducted by a physician.

Plan of Correction: Documentation of th physician examination of child #2 has been documental make sure that the parent provids all documents on time. [sic]

Description: Based on record review, 2 of 2 children's records reviewed did not contain all of the required information Evidence: Child #1 record was missing : 2nd emergency contact physicians information Child #2 record was missing the first date of attendance

Plan of Correction: Provide will make sure that the parent provids child #1 emergency contact and the physizians record on time. [sic]

Description: Based on record review, the provider did receive signed information from parents. Evidence: Child #1 file did not contain the liability insurance form. The provider stated that the paperwork was sent with the parent but had not been returned yet.

Description: Based on record review and interview, the provider did not obtain documentation that a child has been adequately immunized according to the Code of Virginia. Evidence: Child #1 began care on 9/30/2015 and did not have an immunization record on file. The provider stated that the family was new to the area and has not yet provided the documentation.

Plan of Correction: Immunization record of child 1 has been recorded on file.

Description: Based on record review, 2 of 3 household members did not have required Tuberculosis Screening form. Evidence: Household members #1 and #2 are over the age of 18 years and are required to have a Tuberculosis Screening and documentation was not available in the household records.

Description: Based on record review, the provider did not complete the required training hours during the licensing year. Evidence: The provider has documentation for 9 total training hours when 16 hours are required.

Description: Based on record review, the provider did not maintain Household Member #1 record. Evidence: Household Member #1 Does not have Sworn Statement and Criminal History Documentation upon turning 18 years of age, Household member #1 last had Central Registry check on 6/15/2011
