Technical Assistance:

An unannounced monitoring inspection was conducted October 20, 2015 from approximately 9:10am to 11:30am to determine the provider's compliance with licensing standards. This inspection was also conducted to monitor the provider?s probationary status per the Special Order issued in September 2015. There were nine non-resident children and two resident children present during the inspection. Two caregivers were supervising the children for a total of 27 points. Children were observed having their diapers changed, eating, coloring pictures of leaves, playing with toys, watching children's programs on T.V. and singing songs. The provider?s home has three primary rooms for the children to use along with a bathroom. Lunch consisted of hotdogs and buns, applesauce, peas and milk. The hotdogs are cut so they are not a choking hazard. During the inspection the emergency supplies were checked including the first aid kit, flashlight, and weather band radio. Documented practiced emergency drills, injury reports, menu, daily attendance and staff training hours were also reviewed during the inspection. The provider stated medication is not administered at this time. Transportation is not provided. Three children?s and two caregivers? records were reviewed during the inspection. Description: Based on review of two caregivers' records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Caregiver #2 (hire date 10/19/15) did not contain documentation of two or more references prior to the caregiver beginning employment.

Plan of Correction: Per provider - I completed the reference checks but have not added the information to the record. I will add the references to the record.

Standard #: 22VAC40-111-170-B
Description: Based on review of two caregivers' records, one caregiver did not obtain a tuberculosis screening within the required time frame. Evidence: During the inspection, the licensing inspector reviewed the record for Caregiver #2 (hire date 10/19/2015). The record contained a tuberculosis screening dated 04/17/14. The tuberculosis screening needed to be completed within 30 days of beginning employment.

Plan of Correction: Per provider - I will have the caregiver obtain an updated tuberculosis screening.
