Three children were in care during the inspection. One household child and three day care children. Total points were eight. Children were observed during free play and snack. The provider is not current in MAT. Inspection. time 10:00am-11:30am
Standard #: 22VAC40-111-60-B Description: Based on review not all children's records were complete. Evidence: Of three children's records reviewed Child A, with a start date of 9/1/15 was missing two addresses for the emergency contacts, Child B, with a start date of 9/1/15 was missing the insurance policy number. Plan of Correction: I will ask the parents for the missing information.
