Comments:
Today an unannounced renewal inspection was conducted with the Provider-Mrs. Nguyen this visit took place in the morning between 10:15-1:30 there were 7 children with the Provider and 1 assistant for a total of 18 points within the supervision guidelines. The children were observed having morning free play, story-time, reading books, blocks, lunch, dessert, and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu, attendance record and emergency procedures were observed during this inspection.

Violations:
Standard #: 22VAC40-111-750-B
Description: Based on observation of the physical plant, a topical ointment was found in the childcare area accessible to children. Evidence: A diaper cream ointment was found on the floor accessible to children.

Plan of Correction: Provider immediately removed the ointment from the floor during this inspection.

Standard #: 22VAC40-111-120-B
Description: Based on review of the caregiver records, one record did not include documentation of a name, address and phone number of an emergency contact person, and 2 or more references. Evidence: Caregiver #2's record did not include the above documentation.

Plan of Correction: Provider stated that she will have them write down the emergency contact person information and is waiting for one more reference.

Standard #: 22VAC40-111-180-A
Description: Based on review of the caregiver records, one record did not include documentation of an updated tb screening two years from the date of the last screening. Evidence: Household member #2 did not have an up to date tb screening.

Plan of Correction: Provider stated that they went to hong kong and just came back last year but will update it immediately.

Standard #: 22VAC40-111-530-F
Description: Based on observation of the children during nap time, one infant was observed with a blanket under him. Evidence: An infant was placed in the crib with a blanket.

Plan of Correction: Provider immediately removed and will remind staff that it is not allowed.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on review of the provider records, one caregiver record did not include documentation of a criminal record and central registry check before 30 days of employment ends. Evidence: Caregiver #2's who was hired on 9/2 did not have documentation of criminal record check and cps background check results.

Plan of Correction: Provider stated that she sent them off, and provided a copy of the certified mail receipt dated 10/10/15. awaiting results back

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of the provider records, one household member record did not include documentation of an updated criminal record, and central registry check three years from the date of the last screening. Evidence: Household member #1,#2 & #3's record did not include an updated Criminal record, cps check and sworn disclosure statement since 4/15.

Plan of Correction: Provider stated that she sent them off, and provided a copy of the certified mail receipt dated 10/10/15. awaiting results back and will make sure that they are kept updated at all times.
