54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitor inspection was conducted on 10/27/2015. Upon arrival, there were a total of ten (10) children ages 6 months to 5 years old, the Provider and one (1) assistant present. Children present totaled 27 points under direct supervision of 2 caregivers. Provider does not administer medication and does not transport children. Provider continues to participate in the USDA Child Care Food Program. Children and staff records were reviewed during the inspection including three (3) records for recently enrolled children, and two (2) staff/household records. Previous violations were reviewed and have been corrected. Children were observed during various activities. Risk Assessment & areas of non compliance were reviewed with the Provider on 10/27/2015.
Standard #: 22VAC40-111-600-B Description: Based on observations, the Licensee failed to ensure infants & toddlers spend no more than 30 minutes of consecutive time during waking hours confined in a high chair. EVIDENCE: 2 toddler children were confined in infant/toddler high chairs for 40+ minutes. from 2:34pm until 3:25pm Plan of Correction: Provider will use confined areas for eating only.
Standard #: 22VAC40-111-690-D Description: Based on observations, the Licensee failed to sanitize diapering surfacing after each use. EVIDENCE: Diapering changing pad was not sanitized after each use. LI Anderson observed diapering activities for 4 children. Plan of Correction: Provider will review diapering procedures with assistant.
Standard #: 22VAC40-111-690-E Description: Based on observations, the Licensee failed to use a leak-proof or plastic lined storage system that is foot operated to dispose soiled disposable diapers & wipes. EVIDENCE: Soiled disposable diapers & wipes were put in a plastic bag hanging from the door knob on the bathroom door Plan of Correction: Provider will use foot operated trash can for soiled diapers & wipes.
