Comments:
The inspector arrived at 10:00 AM and departed at 11:30 AM. Five children, between the ages of two and four year, totaling 10 points, were in care with one caregiver. A sampling of three children's records, three caregiver/household records and two family pet records were reviewed. Free play and morning lessons were observed. Outdoor play was not observed due to inclement weather. No children's injuries were reported. It was reported that no medication has been administered to children in care. An exit interview, during which the provider's plan of correction for the violation cited was obtained,was conducted prior to the concluding the inspection. Standard #: 22VAC40-111-180-A
Description: Based upon review of three caregiver/household records, the provider has not ensured that tuberculosis screenings are obtained every two years. Evidence: The most recent tuberculosis screening for caregiver/household member 3 is dated June 2013.

Plan of Correction: A repeat tuberculosis screening will be obtained.
