Discussed with the provider: When the child returns from vacation, all new forms will be needed. The full emergency preparedness plan needs to be completed.
Initial Licensing Inspection was conducted with the provider and her husband. The inspection began at 9:15 and ended at 10:30. The provider's 12 year old and 8 month old and one 2 year old day care child were present. The day care area is set up with sufficient space for 9 children and supplies and equipment for all ages. The provider has a record keeping system in place to care for family records and children's records. Energency escape drills have been started. Maps are posted. Standard #: 22VAC40-111-90-C Description: Based on a review of 4 children's files,it was determined that 1 child needed updated immunizations. Evidence: Child #1, does not have an updated immunization record. Plan of Correction: The parent has been asked for the new form.
