Technical Assistance:
Consultation was given on correctly completing the Sworn Disclosure Statement (SDS) form. One household member is missing a signature and another household member needs to read the form and understand the questions before completing the statements to accurately reflect his history.

An unannounced monitoring visit was conducted to modify the address of the Family Day Home license. The modification visit was conducted on two days as the home address was previously licensed with another provider. The Licensing Inspector for the previous owner accompanied the current LI to verify the license was no longer with the previous tenant. The tenant moved at the end of September and the provider moved in to the home in early October. Changes were confirmed and the previous licensee will be closed. Four children were present both days for eight points with the provider which is within ratios for supervision. Home was cleaned up and bought into compliance. Observed children hand washing, free play and transition to rest time. Four cribs were present and all four meet the CPSC safety standards as all were manufactured after 6/2011. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 10 calendar days from date of receipt. The Provider's responses for the "Description Of Action To Be Taken" were not received as of 11/4/2015 and will not appear on the Violation Notice. Description: Based on provider statement, a written emergency plan has not been written for the new location of the family day home. Evidence: The plan needs to be site specific for the new family day home address for relocation, shelter in place and emergency exits.

Description: Based on record review, repeat background checks were not submitted prior to the three year expiration for several household members. Evidence: Central Registry checks have expired before the new ones were submitted for three houshold members. The CPS checks expired in July 2015.
