54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted 10/27/2015 on from 10:00am-11:30 am . There were 7 children present observed with the provider and 2 assistants . The children were observed in a variety of activities to include: free play, diapering, hand washing and potty time, blocks and singing songs. The following areas were inspected during today?s visit: physical environment, emergency preparedness plan, emergency drills, records ( children and caregiver) emergency supplies and posted documents. Standard #: 22VAC40-111-60-B Description: Based upon record review, required documentation was found to be incomplete or missing. Evidence: Child A's record is missing documentation of first date of attendance and insurance policy number. Child B's record is missing an updated medication administration policy signed. Child C''s record is missing documentation of first date of attendance, complete emergency contact information for contact #1 and #2. Child E's record is missing medical insurance policy number. Plan of Correction: Will have parents complete information at pick up.
Standard #: 22VAC40-111-80-A Description: Based upon record review, required documentation was found to be incomplete. Evidence: Child D's record is missing documentation that proof of birth was witnessed by the provider within 7 days of first date of attendance. Plan of Correction: Will have parents bring to view again.
Standard #: 22VAC40-111-90-A Description: Based upon review of children's records, required documentation was found to be missing. Evidence: Child A start date October 2015, does not have a record of immunization on file. Child B start date October 2015 does not have immunization record on file. Plan of Correction: The child does have a form from doctors office in file but it is not signed . Will have parent provide correct form.
Standard #: 22VAC40-111-180-A Description: Based upon record review, household member is missing required documentation. Evidence: Household member 1 has an expired TB test on file ( expiration 07/2014). Plan of Correction: Will have her go get TB screening.
Standard #: 22VAC40-191-40-D-1-c Description: Based upon record review, require record updates are not on file. Evidence: Household member #1 has documentation that CPS forms were submitted for repeat checks and is stamped with the date 10/10/2014. However, there are no findings attached. #1 also has a Criminal History record check on file submitted to the Fairfax County police Department on 09/16/2015. ( this is not the correct form.) Plan of Correction: Provider will resubmit to obtain correct forms.
