Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:10am - 12:15pm. There were twelve children in care for a total of thirty three points with three caregivers. The children were observed at free play and watching cartoons. One caregiver and two children's records were reviewed. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-60-B
Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed two children's records. One of two records reviewed (child record #1) did not address allergies and any chronic physical problems.

Plan of Correction: The provider will ask the child's parents for this information today. The provider will review the children's records quarterly to ensure all information is complete.

Standard #: 22VAC40-111-90-A
Description: Based on record reviews, the family day home failed to obtain immunization documentation before attendance. Evidence: 1. The inspector reviewed two children's records. One of two records reviewed (child record #1) did not contain documentation of immunizations and child's date of enrollment was 11/09/15.

Plan of Correction: The provider will remind the child's parents that documentation of immunizations is needed for the child's record. In the future, the provider will obtain immunization information before the child comes into care.
