Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, and activities were observed. Children were observed; and staff and children files and other documentation were reviewed. Six (6) children were found to be in care totaling 12 points. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:30am and concluded at 9:45am.

Violations:
Standard #: 22VAC40-111-140-B-3
Description: Based on review of training certificates and discussions with the provider compliance was not met with this standard as required. Evidence: The providers and assistants CPR certification had expired as of 10/19/15, and had not yet been renewed as required. Therefore, there is no one present with the children who is currently certified in CPR as required.

Plan of Correction: Contact will be made immediately to obtain the certification for CPR and FA. In the future, both will be renewed prior to expiration to maintain compliance as required, and for the continued safety of the children in care.

Description: Based on review of training certificates and discussions with the provider compliance was not met with this standard as required. Evidence: The providers and assistants First Aid certification had expired as of 10/19/15, and had not yet been renewed as required. Therefore, there is no one present with the children who is currently certified in First Aid as required.

Plan of Correction: Contact will be made immediately to obtain the certification for CPR and FA. In the future, both will be renewed prior to expiration to maintain compliance as required, and for the continued safety of the children in care.
