Technical assistance today included required postings; organization of paperwork; physical environment.

An unannounced monitoring inspection was conducted today from 11:00 - 1:30 pm. There were five children in care with the provider for 14 points. Standard #: 22VAC40-111-100-A
Description: A review of records today found that there was no physical examination available for one children in care over 30 days. Evidence: There was no physical exam today for Child # 3, who began care 8/15.

Plan of Correction: I will find it or I will talk to the mother.

Standard #: 22VAC40-111-60-B
Description: A review of five children's records today found that some required information was missing. Evidence: There was no number for a medical insurance policy for Child # 1; and there was no documentation of annual review of information for Child # 1 and 2.

Plan of Correction: I will talk to the parents today.

Standard #: 22VAC40-111-90-A
Description: A review of records today found that there was no documentation of immunizations available today for one child in care. Evidence: There was no shot record available today for Child # 3.

Plan of Correction: I will find it or I will talk to the parent.

Standard #: 22VAC40-111-240-A
Description: During today's inspection, some areas were observed to not be maintained in a safe condition. Evidence: On the stairs going down to the child care area, decorative stair covers were not securely fastened to many of the stairs, posing a tripping hazard for anyone going up or down the stairs. Two cribs had small accessible strings on the underside of the crib, accessible to toddler-aged children. (removed during inspection)

Plan of Correction: They will be attached better with tape or carpet tape.

Standard #: 22VAC40-111-250-A
Description: During today's inspection, hanging items were observed to be within reach of children under age five. Evidence: The cord from a camera posted on the wall in two areas was observed to be within reach. In one area, it was accessible from a crib. In another area, it was accessible near the children's bookshelf.

Plan of Correction: I will move the crib today and make the cords so the children cannot reach them.
