Comments:
An unannounced monitoring inspection was conducted today, 12/10/15. The violations from the previous inspection have been corrected. A violation notice was completed regarding the violation found during today's inspection. Upon arrival to the family day home some of the children were watching cartoons, others played with the toys. The children changed activities throughout the day. Lunch, hand washing, diapering were also observed.
There are nine children in care totaling 25 points with two caregivers.

Inspection began: 10:10 am and ended 12:00 noon.

Violations:
Standard #: 22VAC40-111-690-D
Description: Based on observation during the inspection the provider failed to have a diapering surface that is non absorbent. Evidence: 1. The diapering surface has several holes in it which prohibits it from being non absorbent.

Plan of Correction: The provider will purchase another diaper mat to change diapers on.
