Reviewed with the provider substitute provider and assistant requirements.

An unannounced renewal study was conducted today with the provider, who was initially present with four children, midway through the inspection a fifth child arrived for a total of 13 points. The children were finishing breakfast at the beginning of the inspection, breakfast consisted of french toast, bananas, cheerios and milk. No children had medication on site. The provider has obtained the required 16 hours of annual training. Three adult files and five children's files were reviewed today. The provider's First Aid and CPR certification is through American Red Cross and is due to expire on 6/21/16. Arrival time: 8:35 am Departure time: 9:30 am

Description: Based on a review of record keeping and an interview with the provider, it was determined that a household member has not obtained a tuberculosis test/screening. Evidence: Person A does not have a tuberculosis test/screening.

Plan of Correction: I will have Person A obtain a tuberculosis test/screening.
