Technical Assistance:
Consult provided on shelter-in-place drills, crib requirements. Cribs manufactured before June 28, 2011 must meet Consumer Product Safety Commission standards. Please forward the required documentation for crib compliance within 30 days (1/29/16).

Comments:
An unannounced renewal inspection was conducted with the provider and assistant beginning at 8:45 a.m. and ending at 11:00 a.m. There were seven children in care ranging in age from six months to four years for a total of twenty-two points. Children were observed watching an age appropriate television program, arriving to the family day home and preparing for breakfast. Breakfast was mini pancakes, banana and milk. Handwashing was observed. After breakfast, children had free play. After play, children were observed during transition and cleaning up from free play. Children then watched an age appropriate television program while snack was prepared. Snack today is apple slices, ritz crackers and milk. Five children's records, two provider records and three household member records were reviewed. The provider completed 17.5 hours of training for the license year. The assistant completed sixteen hours of training for the license year. The provider is Medication Administration Training (MAT) certified, however there were no medications on site. Standard #: 22VAC40-111-830-B
Description: Based on review of documentation and interview with the provider, it was determined that shelter-in-place drills were not practiced a minimum of twice per year. Evidence: There was no documentation of shelter-in-place drills.

Plan of Correction: Shelter-in-place drills will be practiced at least two times per year.

Standard #: 22VAC40-111-920
Description: Based on observation, the menu for 12/14/15-12/18/15 was not complete. Evidence: There were no meals/snacks documented for Friday, 12/18/2015.

Plan of Correction: The menu will be updated immediately. The menu will be completed for the entire week

Standard #: 22VAC40-111-240-A
Description: Based on observation and interview with the provider, all equipment in the family day home was not maintained in safe condition. Evidence: A Delta brand crib had a manufacture date of 12/13/2006, two Ikea cribs had manufacture dates of 12/2009, a Child Craft brand crib had no manufacture date and no documention that it met current CPSC standards and a PJ Kidds LLC brand crib had a manufacture date of November 2003.

Plan of Correction: I plan to remove two of the cribs from the nap room because they are not needed. The remaining three manufacturers will be contacted for the required documentation. If the documentation cannot be obtained, then new cribs will be purchased.
