54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted today from 9:15am-10:00am. There were 6 children(20 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. There was an adequate supply of materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-110-B Description: Based on record review, a child's physical examination report was not signed by a licensed physician, the physician's designee, or an official of a local health department. Evidence: The physical examination report observed for Child 4 was not signed by the child's physician. Plan of Correction: I will ask today.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 1. Only one of two required emergency contacts was observed documented for Child 1 and Child 2. 2. The father's work information was not observed documented for Child 2. Standard #: 22VAC40-111-90-A Description: Based on record review, before a child may attend the family day home, the provider did not obtain documentation that the child had been adequately immunized. Evidence: Documentation of an immunization record was not observed for Child 4, who enrolled November 5, 2015. Plan of Correction: I will ask today.
