63.2 Liability Insurance Disclosure

Today an unannounced monitoring visit was conducted with the provider. This visit took place in the morning between 9:00-10:45 there were 3 children present with the Provider and 1 assistant for a total of 10 points within the supervision guidelines. The children were observed having free play, arriving to the family day home, having breakfast, lunch and in transition for morning nap. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu and emergency procedures were observed during this inspection. Description: Based on review of the children records, one record did not have documentation of proof of identifaction within seven days of enrollment. Evidence: Child #1's did not have documentation of proof of id (enrolled 9/15)

Plan of Correction: Provider stated that the parent is in the process of requesting a new copy of the birth certificate.

Standard #: 22VAC40-111-680-B
Description: Based on observation of the caregivers during feeding, one child's hands were not washed. Evidence: A caregiver placed a child in a high child and did not wash there hands before eating.

Plan of Correction: Caregiver immediately removed the child from the high chair and went to wash the hands.
