Comments:
The licensing inspector conducted an unannounced monitoring inspection on Wednesday, December 2, 2015 from 1:20pm to 2:45pm. The children were resting upon the inspector?s arrival, and they were later observed eating lunch. Lunch consisted of: Ham on a roll, green beans, applesauce, and milk. Points: Today, there were three children, totaling six points, present with two caregivers. The caregivers were observed fixing lunch and guiding the children through the daily routine. Two children?s records and two caregivers? records were reviewed during the inspection. Last emergency drill: 11/25/2015 Last shelter-in-place drills: 7/17/2015 & 10/23/2015 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records on 12/02/2015 the provider did not ensure that one of two records reviewed contained a physical examination within 30 days of attendance. EVIDENCE: The record of Child #1, who started at the family day home on 10/29/2014, does not contain documentation of a physical examination. The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Plan of Correction: Will let parent know today to get physical completed.
