- Technical Assistance:
Consult provided on hand washing, diapering, shelter-in-place drills and stairs. When a child's clothing or diaper becomes wet or soiled, the child shall be cleaned and changed immediately. No child under the age of two years old shall have access to stairs. If the family day home is constructed so that hardware-mounted or hardware-mounted walk-through safety gates cannot be installed at stair openings, the provider still must ensure that young children do not have access to stairs.
An unannounced renewal inspection was conducted beginning at 11:00 a.m. and ending at 2:00 p.m. with the provider. There were four children in care ranging in age from eighteen months to four years old, for a total of nine points. Children were observed having free play, coloring and during snack. Snack was ritz crackers, string cheese and milk. Hand washing was observed. The weekly menu was up to date and posted. Four children's records, one provider record and four household member records were reviewed. The provider completed sixteen hours of training for the license year. Emergency drills were reviewed. The provider is not Medication Administration Training (MAT) certified and cannot provide medications to children in care. Standard #: 22VAC40-111-830-B Description: Based on review of documentation and interview with the provider, shelter-in-place drills were not practiced a minimum of twice per year. Evidence: There was no documentation of shelter in place drills. Plan of Correction: Shelter in place drills will be practiced at least twice per year and documented.
Standard #: 22VAC40-111-960-C Description: Based on observation, the provider failed to ensure that the safety straps on a highchair were fastened. Evidence: An 18 month old child was observed in a high chair without the safety straps fastened. Plan of Correction: Safety straps will be fastened when using the high chair.
Standard #: 22VAC40-111-960-E Description: Based on observation, there was more than one child in care with a bottle and the provider failed to ensure that bottles were labeled with the child's name and dated as required. Evidence: Three bottles were observed that were not labeled with the child's name or dated. Plan of Correction: I will ask all parents to label bottles that are brought to the daycare with the child's name and date.
Standard #: 22VAC40-111-100-A Description: Based on review of documentation, the provider failed to obtain documentation of a physical exam within 30 days of attendance. Evidence: Child A, enrolled 8/27/2015, did not have documentation of a physical in the record. Plan of Correction: The parent will be asked for this information.
Standard #: 22VAC40-111-430-A Description: Based on observation, a child under the age of two years of age had access to stairs. Evidence: The basement stairs where the childcare area is located had no barrier and an 18 month old child was observed accessing the stairs. Plan of Correction: A gate will be put at the bottom of the stairs so children do not have access to the stairs.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of documentation, there was no documentation of sworn statement or affirmation in the records. Evidence: Provider, household member a, b, and c did not have documentation of a sworn statement in the file. Plan of Correction: This information has been misplaced. We will locate this information as soon as possible. If the forms can't be found, we will complete new forms.
