An unannounced renewal study was conducted today. There are 2 children present (5 points) with one adult caregiver. 1 adult and 3 children's files were reviewed. See violation notice for missing information. Emergency evacuation drills, menu, and emergency supplies are all in compliance. Children were busy with a variety of activities. Standard #: 22VAC40-111-100-A Description: Based upon a review of files, children's files do not contain documentation of physicals past 30 days of enrollment. Evidence: 3 of 3 files reviewed were missing documentation of completed physicals. (Child 1 (Date of Enrollment (DOE): 8/18/15), Child 2 DOE: 8/18/15), (Child 3 DOE: 8.19/15). Plan of Correction: I will ask all of the parents to obtain physicals.
