An unannounced monitoring inspection was conducted on January 11, 2016 from approximately 11:00 AM to 1:15 PM. Provider and two assistants were present with ten children. Children were observed during free play and lunch. Three children?s records and two caregiver records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, children and caregiver records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings
Standard #: 22VAC40-111-760-A-1 Description: Based on observation, provider did not ensure all required components are available for the first aid kit. Evidence: On January 11, 2015, a thermometer was not available for the first aid kit. Provider reported the thermometer had broke and she would buy a new one. Plan of Correction: Provider will purchase a new thermometer.
Standard #: 22VAC40-111-100-A Description: Based on review, provider did not ensure 1 of 3 children's records has a physical examination within 30 days of beginning attendance. Evidence: On January 11, 2015, the record for child #2 (start date November 4, 2013) does not have a physical. Plan of Correction: Provider will ask dad for a physical.
Standard #: 22VAC40-111-210-A Description: Based on review, provider did not ensure 2 of 2 caregivers complete at least 16 hours of annual training. Evidence: On January 11, 2015, the following was found: 1. The record for caregiver #1 has documentation of completing 8 hours of training in 2015. 2. The record for caregiver #2 has documentation of completing 4 hours of training in 2015. Plan of Correction: Provider will ask trainer for documentation of trainings completed in 2015.
