Technical Assistance:
An exit interview was conducted with the provider to discuss the result's of today's inspection. Zoning documentation from the local Planning and Development Office was reviewed and a copy obtained for licensing records.

An unannounced monitoring inspection was conducted today, January 12, 2016 to determine the provider's compliance with licensing standards. The inspection was initiated at 3:20pm and concluded approximately 4:45pm. In attendance were 3 children totaling 6 points being supervised by the provider. Children were observed eating snack (pop tart and milk), interacting with the provider, and dancing to children's songs. Children books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home used by children were inspected with no hazards found. The family day home had the required postings displayed on the bulletin board upon entering the children's room. The home's first aid kit, fire extinguisher, and emergency supplies were inspected. Three children and two provider records were inspected with the majority of the information found, see attached violation notice. The following was also inspected: fire drill log, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information. Standard #: 22VAC40-111-100-A
Description: Based on the inspection of children records, the required physical examination documentation was not present. Evidence: During the inspection of three children records, it was discovered child #2 with the start date of 10/18/15, did not have a physical examination in the record. Licensing standards require the provider to obtain documentation of a physical examination prior to attendance or within 30 days after the first day of attendance.

Plan of Correction: Per provider: I will have the parent to bring a copy of the child's latest physical.

Standard #: 22VAC40-111-170-A
Description: Based on the inspection of provider records, the required TB test has not been completed. Evidence: During the inspection of two provider records, provider #1 did not have an updated TB test in the record, the previous TB test dated 5/14/13 expired 5/14/15; an updated TB test or screening should have been obtained prior to the expiration of the previous TB test.

Plan of Correction: Per provider: I will get an updated TB test by the end of the week.
