A unannounced monitoring inspection was conducted on January 11th 2016. There were 3 children (7 points) in the direct care of 1 provider. Children were observed learning about arctic animals, learning about letters, counting by 5 to 100, practicing Spanish words, playing with an arctic animal scene, drawing, playing with dolls, and singing. 1 provider record, 3 children's records, 1 household record, evacuation drills, the emergency preparedness plan, the physical space, supervision, and first aid/CPR training were observed and inspected. Standard #: 22VAC40-111-100-A Description: Based on record review and interview, 1 of 3 children's records did not contain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Child #1 enrolled on 12/29/2014, but does not have documentation of a physical. The provider stated that the parents are aware that a physical is required, and will be faxing it to the provider shortly. Plan of Correction: Not available online.
