Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:00am - 12:30pm. There was one child in care for a total of two points with one caregiver. The child was observed at free play, having a morning snack and watching cartoons. One caregiver and four children's records were reviewed. Violations from the previous inspection were also reviewed and four were repeated. Compliance for standard 500-C could not be determined today because the ground and outdoor play equipment are covered in snow and ice. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record reviews, the family day home failed to obtain documentation of a physical examination. Evidence: 1. The inspector reviewed four children's records. Three of four records reviewed (children's records #1, #3, #4) did not contain documentation of a physical examination and children have been in care for more than 30 days. Child #1's date of enrollment was 5/21/15, child #3's date of enrollment was 6/1/15 and child #4's date of enrollment was 6/1/15.

Plan of Correction: The provider will remind the parents again that this information is needed for the children's records. The provider will review the children's records and will not file the records until they are complete.

Standard #: 22VAC40-111-60-B
Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed four children's records. Two of four records reviewed (children's records #3 and #4) did not contain the children's medical insurance and policy number and both records were missing an address for one emergency contact.

Plan of Correction: The provider will remind the children's parents again that this information is needed for the children's records. The provider will review the children's records more often to ensure all information is complete.

Standard #: 22VAC40-111-90-A
Description: Based on record reviews, the family day home failed to obtain documentation of immunizations. Evidence: 1. The inspector reviewed four children's records. Two of four records reviewed (children's records #3 and #4) did not contain documentation of immunizations.

Plan of Correction: The provider will request this information again from the children's parents. These children are school age and the provider will suggest that the parents obtain a copy from the school.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the family day home failed to obtain a repeat tuberculosis screening. Evidence: 1. The inspector reviewed one caregiver's record. Caregiver #1's tuberculosis screening was over two years old. The last screening was dated 6/11/13.

Plan of Correction: The caregiver will get a repeat tuberculosis screening next week and made sure these stay current.

Standard #: 22VAC40-111-210-A
Description: Based on record reviews, the family day home failed to have a caregiver obtain 16 hours of annual training.
Evidence:
1. The inspector reviewed one caregiver's record. Caregiver record #1 contained documentation of 13 hours of annual training.

Plan of Correction: The caregiver has training scheduled for February 9. The provider will obtain three additional hours of training this year to make up for the missing hours.
