An unannounced monitoring inspection was conducted on February 10, 2016 from approximately 2:05 PM to 4:30 PM. Substitute Provider and assistant were present with nine children. Children were observed during nap time. Two children records and two caregiver records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, nutrition, children/caregiver, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings.
Standard #: 22VAC40-111-960-E Description: Based on observation, provider did not ensure bottles are labeled with the child's full name and date when more than one infant is in care. Evidence: On February 10, 2016, Licensing Inspector observed 3 bottles and they were not labeled with the child's name and date. Staff #1 verified that the bottles was for more than one child. Plan of Correction: Providers will make sure the bottles are labeled.
Standard #: 22VAC40-111-120-A Description: Based on 1 of 2 records reviewed, provider did not ensure there were records for each caregiver. Evidence : On February 10, 2016, there was not a record for caregiver #2. Caregiver #1 said that caregiver #2 was filling in for the day. Plan of Correction: The provider will make sure the record is completed.
Standard #: 22VAC40-111-230-B Description: Based on 1 of 1 records reviewed, the provider did not ensure staff met required annual training hours. Evidence: During the inspection conducted on February 10, 2016, the record for caregiver #1 had documentation of 4 hours of annual training for 2015. Plan of Correction: She will complete her 16 hours of training for 2016.
Standard #: 22VAC40-111-510-C Description: Based on observation, provider did not ensure there is at least 12" of space between occupied cots during nap time. Evidence: During the inspection conducted February 10, 2016, Licensing Inspector observed 6 children napping. The 5 out of the 6 cots did not have required space between the children. Plan of Correction: The provider will move the high chairs near to sink to help spacing.
Standard #: 22VAC40-111-530-A Description: Based on observation, the provider did not ensure that cots and mats used by children during the designated rest period have a top cover and a bottom cover or a one-piece covering which is open on three edges. Evidence: During the inspection conducted on February 10, 2016, the licensing inspector observed children with top covers but no bottom covers. Plan of Correction: The provider will make sure the children will have bottom covers.
Standard #: 22VAC40-111-680-B Description: Based on observation, the provider did not ensure the children hands were washed with liquid soap and warm running water after diapering. Evidence: During the inspection conducted on February 10, 2016, the licensing inspector observed after children were diapered the caregiver did not have the children wash their hands. Plan of Correction: The caregiver will make sure children's hands are washed after diapering.
Standard #: 22VAC40-111-690-D Description: Based on observation, the licensee did not ensure the diapering surface is nonabsorbent surface. Evidence: During the inspection conducted on February 10, 2016, Licensing Inspector observed Caregiver #1 diapering child on couch and caregiver #2 diapering child on a yellow blanket on the couch. Plan of Correction: Children will be change on the changing table.
