Comments:
An unannounced monitoring inspection was conducted today, February 11, 2016 to determine the provider's compliance with licensing standards. In attendance were 5 children totaling 13 points being supervised by the substitute provider. Children just completed lunch, which consisted of barbeque chicken, apple sauce, fruit cocktail, biscuit, and milk. Afterward children slept on mats with the appropriate bedding. Children books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. The family day home had the required postings displayed on the bulletin board in the children's room. The first aid kit, fire extinguisher, and emergency supplies were inspected. Two children and one staff records were inspected with the majority of the required information found. The following was also inspected: fire drill log, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information. Standard #: 22VAC40-191-40-D-1-c
Description: Based on the inspection of administrative records, the required update was not completed. Evidence: During the inspection of one staff's record, it was determined that the sworn statement or affirmation form dated 12/4/12, expired on 12/4/15. This form is required to be updated every three years with the appropriate information completed.

Plan of Correction: Per provider: I will have the staff member to complete a new sworn statement form and place it in the record for review.
