An unannounced renewal study was conducted from 4:00 p.m.-5:00 p.m. with the provider and assistant. Three children, ages 5, years, 6 years, and 8 years were in the care of two caregivers for a total of 3 points. Children were observed doing homework, playing outside, playing in the pretend kitchen, and sewing. Afternoon snack today was graham crackers and milk. The last emergency escape drill was 1/27/16. The provider is current in first aid and CPR. Three household member records and one children's records were reviewed and in compliance. Required information was posted. Description: Based on document review and provider interview, it was determined that there was no documentation that the wood burning fireplace had a current annual inspection. Evidence: The last documented inspection of the fireplace was in 2014.

Plan of Correction: The fireplace was inspected, but I am unable to locate the paperwork. I will contact the inspector to obtain another copy of the inspection report.
