An unannounced renewal inspection was conducted today, 2/22/16. The violations from the previous inspection have been corrected. A violation notice was completed regarding the violations found during today's inspection, there is one repeat violation. Upon arrival to the home the two children in care were napping. Records were reviewed for compliance and a walk through the house was also conducted. Should you have any questions please do not hesitate to call. Inspection began: 11:30 am to 2:30 pm.
Standard #: 22VAC40-111-60-B Description: Based on review of the children's records during the renewal inspection on 2/22/16 the provider failed to get all the required information. Evidence: 1. Children's records 9, 11, 12 do not have an address or phone number for the physician listed. Plan of Correction: The provider will have the parents get this information for her. If they do not supply this then she will look online to see if it is listed.
Standard #: 22VAC40-111-140-B-4 Description: Based on review of records the provider failed to have a current first aid certification. Evidence: 1. The provider does not have documentation of a current first aid certification. Plan of Correction: The provider stated that she completed this certification about two years ago and was not given the certification card. The provider has attempted to call the instructor to check on the card and no calls have been returned. The provider will contact the local rescue squad to see if they have any trainings coming up that she can attend.
Standard #: 22VAC40-111-260-E Description: Based on observation of the house the provider failed to empty a mop bucket filled with water. Evidence: 1. Located in the kitchen area, accessible to the children, is a mop bucket filled with water. The water was placed in the bucket during the morning hours. Plan of Correction: The provider will begin today mopping directly after filling the bucket and then empty the water out.
Standard #: 22VAC40-111-460-C Description: Based on review of the household animal files the provider failed to have an up to date rabies shot for the dog. Evidence: 1. The providers dogs rabies shots expired 2/7/16. Plan of Correction: The provider will get these shots this weekend.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of records the provider failed to get the repeat background checks. Evidence: 1. Staff # 1, 2, 3 have a criminal history record that expired 5/23/15 2. Staff # 1, 2, 3 have a central registry checks that expired 5/25/15. Plan of Correction: The provider stated that the background checks have been mailed off last week. The provider will ensure that in the future all repeat checks are conducted prior to the background checks expiring.
