Comments:
An unannounced renewal inspection was conducted on February 9, 2016. There were five children present during the inspection with the provider totaling 11 points. No medications are being administered in the family day home at this time. Two children's records and the provider's record were reviewed during the inspection. The outdoor play area is not being used at this time.

Standard #: 22VAC40-111-770
Description: Based on observation and inspection of the family day home, the provider did not have a working portable battery-operated weather band radio and extra batteries that were kept in a designated area and available to caregivers at all times. Evidence: During the inspection conducted on February 9, 2016, the provider did not have batteries for the battery-operated weather band radio.

Standard #: 22VAC40-111-830-A
Description: Based on review, the provider did not ensure that emergency evacuation procedures were practiced monthly with all caregivers and children in care during all shifts that the children are in care. Evidence: During the inspection conducted on February 9, 2016, the licensing inspector reviewed documentation that indicated emergency evacuation procedures had not been practiced the following months: September, October, November, December (2015) and January 2016.

Description: Based on observation and inspection of the family day home, the provider did not ensure that when meals or snacks are provided by the family day home, the menu was dated for the current one-week period. Evidence: During the inspection conducted on February 9, 2016, the most recent CPR certification for the provider was dated 5/4/13 and had an expiration date of 5/4/15.

Description: Based on observation and inspection of the family day home, the provider did not ensure that each child shall be provided with an individual crib, cot, rest mat, or bed for resting or napping. Evidence: During the inspection conducted on February 9, 2016, the licensing inspector observed two children sleeping on the floor in the provider's home without a cot, rest mat, or bed.

Standard #: 22VAC40-111-510-D
Description: Based on observation and inspection of the family day home, the provider did not ensure that rest mats that are used must have at least an inch of cushioning. Evidence: During the inspection conducted on February 9, 2016, the licensing inspector observed two children resting on rest mats that did not have at least an inch of cushioning. The rest mats were torn and ripped with the foam padding removed.
