- Technical Assistance:
Any visitors to the home must never be left alone with any children in care. The provider will forward to the Inspector, documentation showing the completion of 7 hours of training courses before the license can be processed for renewal. The provider recently obtained her county special permit, allowing the capacity of her family day home to be increased to 12 children.
An unannounced full renewal inspection was conducted today. The provider and two assistant caregivers were caring for 6 children, totaling 17 points. The children were observed playing with toys, listening to stories, practicing their Spanish color and number words, and preparing for lunch. This inspection was completed between approximately 10:15 am and 12:00 pm. Thank you for meeting with me today. Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that two assistant caregiver records were not complete. Evidence: 1. The file for assistant caregiver #1 does not include: documentation that the provider has verified the employee's age; emergency contact information. 2. The file for assistant caregiver #2 does not include: documentation that the provider has verified the employee's age; a second reference check. Plan of Correction: I will make sure the files are updated and complete.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include a registration form indicating all required parent contact information, insurance information, and emergency contact information. 2. Child record #2 does not include the parent's medical insurance policy ID number; the parents annual review of the child's contact information. 3. Child record #3 does not include a signed Medication Policy document. Plan of Correction: Each parent will be asked for the missing information.
Standard #: 22VAC40-111-200-A Description: Based on a review of caregiver training records, it was determined that the provider did not conduct orientation training for each assistant caregiver as required. Evidence: Each of the current assistant caregivers began employment in January, 2016. The provider has not conducted orientation training with either of these employees. Plan of Correction: I will do the full orientation training with both of them and place the documentation in each file.
Standard #: 22VAC40-111-520-A Description: Based on observations, it was determined that the provider does not have an individually assigned crib for each infant. Evidence: It was reported to the Inspector that one infant in care (age 11 months) does not have an individually assigned crib because she does not sleep for long periods of time while in care. The provider has created a schedule of nap times for this child. When this child is not using her crib, a different child uses the crib. However, the provider does not change the sheets in between each use by each child. Plan of Correction: I have more than enough cribs because my older children usually sleep on rest mats, but they haven't been using the rest mats since moving the nap area to a different room. I will return to the routine of having the older children sleep on rest mats and each infant and young toddler will have his/her own crib.
