Comments:
An unannounced monitoring visit and home tour was conducted from 9:15am-10:20am. At the time of the inspector's arrival there were 5 children ages 6mos-3yrs, totalling 13 points in care with the licensee. Children were observed participating in free play, and toileting/handwashing. A sample of 3 children's records and 1 staff record and 1 household member record were reviewed. Medication is not administered at this site. There were no reported injuries since the last inspection. Areas of noncompliance are identified on the violation notice and were discussed with the provider during the exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review it was determined that the provider did not ensure that the children's records contained all of the required components. Evidence: The record for child #3(enrolled 10/19/15) did not contain the medical insurance policy number.

Plan of Correction: The provider contacted the parent via phone and corrected this deficiency during the inspection.

Standard #: 22VAC40-111-350-E
Description: Based on observation, it was determined that the provider did not ensure that hot water taps available to children shall be maintained within a range of 105-120 degrees Fahrenheit. Evidence: The hot water was measured 125.3 degrees Fahrenheit.

Plan of Correction: Children are supervised when using the bathroom and only use the hall bathroom, which takes a very long time for the water to heat up. Provider will adjust the temperature and test weekly to ensure it is below 120 degrees Fahrenheit. Provider will also look into replacing the hot water heater.
