Comments:
An unannounced monitoring inspection was conducted on February 9th 2016 between 12:30pm and 2:15pm. There were 13 children in the direct care of 2 staff. Children were observed napping, being changed, and preparing for activities. The older group of children was napping and the younger children were active and awake. Household records, children's records, staff records, the physical space, and the evacuation drill were observed and inspected. Violations were observed and are noted on the violation notice. Description: Based on record review, 3 of 3 records did not contain documentation that the parents or guardians reviewed the emergency contacts on file.

Plan of Correction: Schedule review dates with parents

Standard #: 22VAC40-111-100-A
Description: Based on record review, 1 of 3 children's records did not contain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Child #3 did not have documentation of a physical on record.

Plan of Correction: We have schedule a date to review the files with parents

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider failed to obtain a current Report of Tuberculosis Screening form from each caregiver or adult household member every 2 years from the date of first screening. Evidence Household Member#1 last had a Tuberculosis screening on 6/5/2013

Plan of Correction: I have provided a copy of TB that was done but not on file at the time of visit.

Description: Based on record review, 4 of 4 adult records did not have repeat background checks before threes years since the dates of the last checks. Evidence: Household Member#1 : Criminal History Check was last completed 11/28/2012, Central Registry Check was last completed 12/15/2012 Household Member #2: Central Registry Check was last completed on 12/15/2012 Staff Member #1: Sworn Statement was last completed on 9/24/2012, Criminal Record Check was last completed 11/28/2012, Central Registry Check was last completed 12/15/2012 Staff Member #2: Criminal Record Check was last completed 11//28/2012, Central Registry Check was last completed 12/10/2012

Description: Based on record review, the provider failed to obtain a criminal history record check within 30 days of a person in the home becoming 18 years of age. Evidence: Household member #2 does not have a criminal history record check on file and turned 18 years of age on 6/6/2015. This is a repeat violation and was previously cited on 7/20/2015.

Plan of Correction: I have ordered them

Standard #: 22VAC40-191-40-D-5-a
Description: Based on observation, the provider failed to complete a Child protective services central registry check within 30 days of a child residing in the home becoming 14 years of age. Evidence: Household member #3 became 14 years of age on 4/23/2014. This is a repeat violation that was previously cited on 7/20/2015

Plan of Correction: I have ordered them.

Standard #: 22VAC40-80-120-A-4
Description: Based on observation the provider failed to adhere to the terms of the license in regards to the maximum number of children who may be in care at any time. Evidence: Upon arrival at 12:30pm the provider had 12 children in care. 9 children were napping on the main level and 3 were playing on the basement level. At 1:35pm a 13th child arrived. The inspector questioned the provider about this and the provider stated that normally the child arrives at 3:15pm and another child leaves at 3:30pm. The provider was informed that this still violates the terms of her license for 15 minutes each time this occurs.

Plan of Correction: Due to weather conditions the 13th child showed up earlier. Usually she comes at 3:30pm only 2 days per week if she needs my service and I have a child that leaves at 3:15pm everyday.
