Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:00am - 10:45am. There were four children in care for a total of thirteen points with one caregiver. The children were observed at free play and watching cartoons. Two adult and three children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child record #3) did not contain the policy number for the child's medical insurance.

Plan of Correction: The provider has requested this information a number of times, but to date has not received it. The provider will request this information again. The provider will review the children's records to ensure they are complete.

Standard #: 22VAC40-111-180-A
Description: Based on record reviews, the family day home failed to obtain a repeat tuberculosis screening. Evidence: 1. The inspector reviewed two adult records. One of two records reviewed (adult record #1) contained a tuberculosis screening that was over two years old. The screening was dated 10/24/13.

Plan of Correction: The provider said the repeat tuberculosis screening will be done tomorrow. The provider will review the adult records to ensure all information is current.
