Technical Assistance:
Consultation was given on using swaddling for young infants (under 6 months) to help with the startle reflex but to discontinue as soon as the child is able to roll over. All children (under 12 months of age) who are present must have an assigned crib available at all times throughout the day.

Comments:
An unannounced monitoring visit was conducted from 9:45-11:15. Nine children were present with three caregivers for twenty eight points which is within ratios for supervision. The home has two rooms for day care with one primarily for infants as well as a bathroom and entry way into the lower level of the home. The backyard play area was not in use today due to the snow and muddy conditions. Lunch is provided from each child's home. Bottles were labeled with child's name and date. Children were observed resting, working on colors and coloring as well as free play and diapering. Areas of non-compliance are identified in this report. Description: Based on provider statement, only one of two shelter-in place drills (SIP) was conducted with children in 2015. Evidence: The provider stated that a second SIP drill was not conducted for the calendar year 2015.

Description: Based on record review, the January 2016 emergency evacuation drill was not documented. Evidence: The provider stated that a drill was conducted January 4, 2016 but was not recorded on the log.

Description: Based on observation, a swaddling blanket was in use for a child that was not appropriate for the child's age. Evidence: Child #2 was is a swaddling sack upon arrival. The child was asleep on a mat face down and was unable to use his hands to push up if needed. The age range on the swaddling blanket was 3-6 months and the child is 11 months of age.

Description: Based on observation, three children were not resting in a crib. Evidence: Three children were napping on mats on the carpet rather than being placed in their crib during AM rest time.
