Standard #: 22VAC40-111-830-D
Description: Based upon record review, required documentation was found to be missing or incomplete. Evidence: Provider missed the January 2016 evacuation drill that is required monthly due to weather.

Plan of Correction: Provider will make sure to complete drills monthly as required.

Standard #: 22VAC40-111-940-B
Description: Based upon observation, temperature of refrigerator was found to be above required level. Evidence: The thermometer in the refrigerator was reading 48 degrees. Provider brought a second thermometer to test refrigerator, and it too read 48 degrees. Maximum temperature allowed is 40 degrees.

Plan of Correction: Provider will fix or replace refrigerator.

Standard #: 22VAC40-111-50-A
Description: Based upon record review, required documentation was found to be incomplete or missing. Evidence: Attendance records were not completed as required daily. Records were missing for 2/1/2016 and 2/2/2016.

Plan of Correction: Provider will complete attendance daily as required.

Standard #: 22VAC40-111-60-B
Description: Based upon record review, required documentation was found to be missing or incomplete. Evidence: Child A-'s record is missing the parent portion of the physical exam form completed and signed within 30 days of first date of attendance. Child B's record is missing Proof of birth documentation witnessed by the provider within 7 days of first date of attendance. (12/3/2015) Child C's record is missing an annual parent review of required information. 1/6/2015 last review.

Plan of Correction: Provider will request required information from parents at pick up this evening.
