An unannounced monitoring inspection was conducted from 2:00 p.m.-3:15 p.m. with the provider and assistant. Three children, ages 2 years old, 4 years old, and 9 years old, were in the care of two caregivers for a total of five points. Children were observed having afternoon snack, watching an ABC cartoon on a tablet, playing with toy cars, and working on puzzles. Afternoon snack today was apples and milk. One staff member's record was reviewed and in compliance. Two children's records were reviewed. The weatherband radio and battery operated flashlight were present and accessible. The last emergency escape drill was 2/22/16. The provider and assistant are both current in first aid and CPR. Description: Based on review of two children's records, the provider did not maintain complete records. Evidence: Child A's (enrolled 8/31/2015) record was missing an emergency contact's street address and listed the provider as the second emergency contact. Child B's (enrolled 1/2016) record was missing date of first attendance and a parent's employment address and employment phone number.

Plan of Correction: The provider contacted the parents during the inspection and will get this information at pick-up.
