Technical Assistance:
Discussion was held on the topics of current enrollment and current staffing. Discussion of the Provider working with Fairfax Office for Zoning in order to increase the posted capacity indicated on license.

Comments:
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 7 children (17 points) with 2 caregivers. A total of 3 children?s files and 6 staff/household member?s files were reviewed. The children were engaged in the following: outdoor play, diaper changing, games, music, song, hand washing, bathroom breaks, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-500-C
Description: Based on observation, a piece of stationary outdoor playground equipment was over a hard surface. Evidence - 1. On the date of inspection (03/07/2016), from under the swings and in front of the slide attached to the piece of stationary outdoor equipment installed in the Family Day Home's (FDH) backyard, the grass has worn away exposing the hard dirt surface underneath. 2. The Provider confirmed this observation on the date of inspection.

Plan of Correction: Grass seed and/or mulch will be purchase to cover these worn areas.
