Technical Assistance:
Discussed with provider: MAT certification renewal, policy for medication, liability insurance information, Asst. file requirements, gate placement for the stairs. Model DSS forms provided.

An Initial inspection was conducted today. There are 4 children (8 points) with one caregiver. 4 adult/resident files and 4 children's files were reviewed. See violation notice form missing information. Children were observed playing with a variety of activities. Cribs assembled for use (not needed right now) have acceptable manufacture dates. Description: Based upon a review of files, one child's files is incomplete. Evidence: Child 1's file is missing complete emergency contact information for 2 contacts.

Plan of Correction: I will ask the parent to complete the information.

Standard #: 22VAC40-111-70-A
Description: Based upon a review of files, children's files do not contain written acknowledgment of policies and procedures prior to enrollment. Evidence: 4 of 4 files reviewed were missing documentation of policies and procedures. Missing information includes but is not limited to: DSS web site address, mandated reporter statement, custodial parent's rights statement, check in and out procedures, decision to administer medication, liability insurance declaration.

Plan of Correction: I will complete the Information for parents model form and have the parents sign them.

Standard #: 22VAC40-111-500-A
Description: Based upon direct observation, part of the fence in the backyard is not a non-climable fence, that is within 30 feet of a busy street.

Plan of Correction: I will purchase the plastic stirps to make the fence nonclimbable.
