Comments:
Licensing Inspector arrived at 11:45 AM and departed at 12:50 PM. There were three children, an infant and two preschoolers (8 points), in care with one caregiver. Children were observed participating in free play. The sample size was three children's records. A violation was noted in the area of supervision. Response to the violation was received on site during the inspection.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of three children's records, it was determined that the provider did not ensure that all required information was present in each child's record. Evidence: The record for child #1 (date of enrollment 10/29/15) had only one of the two persons designated to be contacted in an emergency if the parents can not be reached.

Plan of Correction: Provider will obtain information from the parent at pick-up this evening.
