Comments:
An unannounced monitoring inspection was conducted March 11, 2016 from approximately 9:45am to 12:00pm to determine the provider's compliance with licensing standards. There were nine children in the care of two providers for a total of 30 points. A third caregiver arrived during the inspection. Children were observed coming back from a walk, playing outside, playing with toys, taking bathroom breaks, having their diapers changed and preparing for lunch. Lunch consisted of turkey, noodles, peas, fruit and milk. Caregivers were observed interacting with children and responding to the children's needs quickly.

During the inspection the following items were reviewed: emergency supplies including the first aid kit, flashlight, and weather band radio, practiced emergency drills, fire extinguisher, smoke detector, daily attendance, staff training, and parent agreements. Medication is not being administered at this time. Transportation is not being provided. A complete walk through of the house was completed during the inspection. Standard #: 22VAC40-111-120-B
Description: Based on review of two caregivers' records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector reviewed the record for Caregiver #2 (hire date 02/04/2016). The record did not contain documentation of a completed negative tuberculosis screening before coming into contact with the children in care. The provider stated Caregiver #2 had a tuberculosis screening but needs to pick up a copy from the doctor.

Standard #: 22VAC40-111-200-C
Description: Based on review of two caregivers' records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector reviewed the record for Caregiver #2 (hire date 02/04/2016). The record did not contain documentation of the completed orientation that was signed and dated by the provider and caregiver.

Standard #: 22VAC40-111-680-A
Description: Based on observation, the caregiver did not wash their hands after diapering a child.

Evidence:
During the inspection, the licensing inspector observed Caregiver #2 change two infants' diapers. After the caregiver was done changing the first infant, the caregiver was observed setting the first infant down and then picking up the second infant and proceed to change the second infant's diaper. Caregivers are required to wash their hands between each diaper change.

Description: Based on observation, the caregiver did not ensure the diapering surface was cleaned and sanitized after each use.

Evidence:
During the inspection, the licensing inspector observed Caregiver #2 change two infants' diapers. After the caregiver was done changing the first infant, the caregiver was observed setting the first infant down and then picking up the second infant and proceed to change the second infant's diaper. The diapering area was not cleaned or sanitized after each diaper change.

Description: Based on review of two caregiver's records, the provider did not ensure criminal record checks were obtained within 30 days of employment for one caregiver. Evidence: During the inspection, the licensing inspector observed the record for Caregiver #2 (hire date 02/04/2016) did not contain a completed criminal record check. Caregiver #1 stated the request was completed and mailed off but the information has not been returned as of 03/11/2016.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of two caregiver's records, the provider did not ensure central registry findings were obtained within 30 days of employment for one caregiver. Evidence: During the inspection, the licensing inspector observed the record for Caregiver #2 (hire date 02/04/2016) did not contain a completed central registry findings. Caregiver #1 stated the request was completed and mailed off but the information has not been returned as of 03/11/2016.

Standard #: 63.2-1720-F
Description: Based on observation and interview, the provider did not ensure an employee without a completed original criminal clearance did not have direct contact with children without the required supervision.

Evidence:
During the inspection, the licensing inspector observed Caregiver #2 (hire date 02/04/2016) supervising three children in the back playroom of the house. Caregiver #1 and Caregiver #3 were supervising the other six children outside and in the kitchen area. The licensing inspector observed Caregiver #1 check on Caregiver #2 throughout the inspection but Caregiver #1 did not always remain within sight and sound of Caregiver #2. Caregivers are not permitted to work in a position that involves direct contact with a child receiving services until an original criminal history record has been received, unless the caregiver remains within sight and sound / direct supervision of another caregiver for whom a background check has been completed.
