Technical Assistance:
Background checks were discussed. Sixteen hours in training is an annual requirement. Secured gates were discussed.

An unannounced inspection was conducted this morning. There were 5 children present with two caregivers for a total of 16 points. The Provider has children's records available for review. The Inspector observed activities and play. The Provider has MAT certification and CPR/First Aid certification but stated no one needs medication and no one is receiving medication. Standard #: 22VAC40-111-800-B
Description: Based on staff interviews and request for plan, the Provider has not reviewed the emergency plan in the last year. Evidence: 1. Provider stated they will go a another family's home and Inspector asked to see the plan. 2. Provider stated the parents have the plan but she has not updated it.

Plan of Correction: I will do it.

Standard #: 22VAC40-111-960-E
Description: Based on observation and staff interview, one set of labeled bottles brought from the infant's home have not been dated.

Plan of Correction: I tell them to date and will tell them again.

Standard #: 22VAC40-111-60-B
Description: Based on record review of two children's records, the Provider has not maintained current children's files. Evidence: 1. The Inspector reviewed two records and both #1 and #2 had not been reviewed annually by the parent to confirm the information remains current. Child #1's record was signed on February 2, 2015 and Child #2's form was signed on August 18, 2014. 2. Child's #1 record does not include acknowledgement of the Emergency Preparedness plan in terms of evacuation plans.

Plan of Correction: I will update them and give out the Emergency plan.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on interviews and record review, the Provider has not ensured that she and a household member (and her assistant) and her back up assistant have search of central registry and criminal history record check before three years since the dates of the most recent central registry finding and most recent criminal history record check report. Evidence: 1. Inspector asked for the background checks and Provider handed an envelope. The envelope contained background check conducted by Fairfax County Police Department which does not meet requirements for a State background check. Provider stated she had asked and thought this was okay. 2. The CPS paperwork had been filled out but signatures for notary had not been conducted and forms were not complete. The forms had also not been sent out to the CPS registry in Richmond.

Plan of Correction: I will get them for all of us.
