22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was initiated today at 12:00pm. The physical plant, 4 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 12 children, totaling 33 points, under the supervision of 3 caregivers. During the inspection, children were observed playing outside, listening to storytime, cleaning up for and resting at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken, rice, salad, apples and milk. The Provider is currently certified in CPR and First Aid. She is not certified in MAT (medication administration training) at this time, but is considering completing the training. Standard #: 22VAC40-111-180-A Description: Based on file review, 2 out of 4 caregiver/household members do not have a completed TB test/screening within the past two years. Evidence 1. Caregiver/household member #1's most recent TB test/screening was completed on 11/17/13. 2. Caregiver/household member #4's most recent TB test/screening was completed on 11/11/13. Plan of Correction: This will be completed.
Standard #: 22VAC40-111-500-C Description: Based on observation, the outdoor playground equipment did not meet requirements for being used was placed on a hard surface. Evidence: 1. The climbing structure was placed on a hard surface (wood deck), with mats that were placed only under the slide and stairs. The mats are not big enough to cover the fall zones by each slide or steps. Plan of Correction: The Provider will get a foam mat (which she currently uses in the FDH) to be placed under and in the surrounding areas of the playground equipment.
Standard #: 22VAC40-111-540-B Description: Based on observations, an infant walker was being used at the FDH. Evidence 1. A child in care was seen using an infant walker while outside on the deck. Plan of Correction: The Provider removed them during the inspection.
