The provider was present with 6 children at the beginning of the visit. A total of 18 points. The assistant arrived after 45 minutes. The children are in their cribs getting ready to nap. The provider is watching the children and we discussed ways that might get the children to nap, such as turning off lights and maybe playing soothing music. The provider has lots of space for play and toys easily accessible. Please call me at 703-203-8659 if you have any questions regarding this visit. This visit was started 11:50 and ended at 1:15.

Description: The provider failed to have a bottle labeled with the date. Evidence: The provider has a bottle of breast milk that is not labeled with the date.

Plan of Correction: I will make sure it is dated.

Standard #: 22VAC40-111-240-A
Description: The provider failed to ensure that some part of the physical plant is in a safe and operative condition. Evidence: Based on the review of the physical plant the handrail leading down to the basement is loose and secured by a piece of metal wire.

Plan of Correction: I had a workman here who said it was very hard to fix but i will get it fixed.

Standard #: 22VAC40-111-560-A
Description: The provider failed to be in sight and sound supervision of the children at the beginning of the visit. Evidence: When the inspector arrived at 11:50 the provider was outside working at the front walkway of her garden. upon arrival she went inside and the inspectors were let in the basement door by the provider, 6 children were getting ready for nap but none of the children were asleep. The children were left unattended for an undisclosed amount of time. Provider was outside for approximately 2 minute according to the provider.

Plan of Correction: I will not leave them alone , I was just gone for a minute, just to turn of water.

Standard #: 22VAC40-111-570-A
Description: The provider failed to ensure that she was caring for no more than 16 points. Evidence: Based on the inspection the provider was present with 6 children , a total of 18 points. The assistant had left around 11 for an appointment and returned during the inspection at 12:30.

Plan of Correction: I will make sure I do not have over 16 points.

Standard #: 22VAC40-111-690-E
Description: The provider failed to put a trash bag in the diaper pail. Evidence: Based on the review of the physical plant , the diaper pail do not contain a trashbag.

Plan of Correction: Will put a new one in the trash can.
