An unannounced renewal inspection was conducted today from 10:15am-11:50am. There were 11 children(37 points) directly supervised by 3 staff. The home, 3 staff records, 4 children?s records, 7 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, playing outside, being fed and eating lunch. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of children's physical examinations prior a child's attendance or within 30 days after the first day of attendance. Evidence: Documentation of a physical examination was not observed for Child 3 (enrolled February 1, 2016) and Child 4 (enrolled September 7, 2015). Plan of Correction: I will let them know and give them the form.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 1. The parent's signed acknowledgement of the receipt of the "Information for Parents" form was not observed for Child 1 and Child 3. 2. The parent's signed acknowledgement of the receipt of the "Liability Insurance Declaration" form was not observed for Child 2 and Child 3. Plan of Correction: I will give it to them today.
Standard #: 22VAC40-111-140-B-3 Description: Based on record review, the provider did not have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care. Evidence: The CPR certification observed for the provider expired January 11, 2016 and current certification had not been obtained. Plan of Correction: I will call them to make an appointment.

Evidence: The first aid certification observed for the provider expired January 11, 2016 and current certification had not been obtained. Plan of Correction: I will call them to make an appointment.
Standard #: 22VAC40-111-320-B Description: Based on direct observation, electrical outlets did not have protective covers. Evidence: Outlets were observed without protective covers in the children's bathroom and on the upper portion of the wall in the back children's room. Plan of Correction: The outlets were covered during the inspection.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review, the provider did not obtain current background checks before three years since the dates of the previous checks for caregivers and household members. Evidence: 1. The most recent sworn statement or affirmation observed for Household Member E was last completed on October 30, 2012 and expired on October 30, 2015. 2. The most recent search of central registry (CPS) observed for Caregiver A was last completed on June 2, 2012 and expired on June 22, 2015 and Household Member E's was last completed on November 28, 2012 and expired November 28, 2015. 3. The most recent criminal history record check observed for Household Member E was last completed on October 13, 2012 and expired on November 12, 2015. Plan of Correction: I will send them out today for Caregiver A and I have already sent for Household Member E.
