Comments:
An unannounced inspection for license renewal was conducted today. Eight children were present with the provider and assistant. Total 18 points with two caregivers. The children were playing inside, eating lunch and resting. No new children have been enrolled since the last inspection. Caregivers and household member's records were reviewed today. The provider has MAT certification but is not administering any medication at this time. If you have any questions, I can be reached at 703-479-4709.

Description: Based on observation, the outside play area was not maintained in a safe condition. Findings: At the time of the inspection, the inspector observed a large piece of splintered plywood leaning against the fence near the outdoor play equipment.

Plan of Correction: The piece of plywood will be removed from the outdoor play area.

Standard #: 22VAC40-111-500-C
Description: Based on observation, the grass under the swing set has worn away leaving only a hard dirt surface.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review, no criminal record check was obtained for a household member who turned 18 years old in December 2015.

Plan of Correction: A criminal record check for this household member will be obtained.
