An unannounced renewal inspection was conducted today from 10:20 - 12:00. There were 11 children = 25 points with three caregivers. The children played with a variety of toys, did a project, went outside and watched a sign language video. The exit interview was conducted with the licensee. The violation notice will be e mailed for completion.

Standard #: 22VAC40-111-700-A
Description: Based on a review of the medications on site today, the licensee did not use the correct form for one child and did not obtain the correct authorization dates for another child. Evidence - The medication authorization form for child #11 was on a form from a different Child Care program which does not authorize the licensee to administer the medication. The medication authorization for child #9 had incorrect dates for the length of the authorization period on the parent section of the authorization form.

Plan of Correction: provider gave parents of child #11 medication authorization form which was related to her own facility and ask them to fill it out and bring it back for the provider. Provider gave the medication authorization form of child #9 to his parents to correct the date of authorization form.

Standard #: 22VAC40-111-120-B
Description: Based on a review of staff files, the licensee did not provide all of the required information for two staff. Evidence - The records for staff #1 and #2 did not contain their date of employment.

Plan of Correction: provider date the day that staff #1 and #2 start working in her facility
