An unannounced monitoring inspection was done 10/19/2105 beginning at 10:05 A.M. and ending at 12:30 p.m. The licensee and household member was present with the licensee's two infant and three child care children. The points were at 14 and in compliance. The LI reviewed six children's and one staff and a household member's record during the inspection. Observation of the children during: free play, TV time, snack, coloring and preparing for a field trip to visit the Easter Bunny. Discussion about upcoming changes to the licensee's household, and future plans for day care schedule.

Standard #: 22VAC40-111-780-B
Description: Based on review of six records, the licensee failed to ensure that all emergency information. Evidence: 1. The inspector found that child # 2 did not have documentation of an annual update for the child's emergency information. The most recent update was 2/23/2015.

Plan of Correction: The licensee will have the parent to complete the form as soon as the child attends again. All records will be monitored.

Standard #: 22VAC40-111-50-A
Description: Based on interview with the licensee, the licensee failed to ensure that the daily written record of children in attendance was completed. Evidence: 1. Through interviews with the licensee at 11:05 the inspector found that the daily attendance record was not completed within 30 minutes of the children's arrival. There were three children in care. One child was the licensee's grandson and was staying at the home for a visit. The other two children arrived at 9:45 and 10:00 A.M.

Plan of Correction: The written attendance was completed at 11:10 A.M. In the future the attendance will be taken as the children arrive.

Standard #: 22VAC40-111-60-B
Description: Based on review of six children's records, the licensee failed to ensure that all records were complete. Evidence: 1. The inspector found that child # 3 needed complete address for both of the required emergency contacts. 2. Child # 4 did not have the chronic physical problems and allergies completed.

Plan of Correction: The licensee will have the parent to complete the missing information as soon as the children return for care. In the future all records will be monitored.
