Comments:
An unannounced renewal inspection was conducted today at 12:10pm. There were 12 children (32 points) directly supervised by 2 staff, the provider appeared after the start of the inspection and the left to take 4 preschoolers to school. The home, 3 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Inspected medications kept on site for children in care. Children were observed playing freely and finishing lunch. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-111-60-B
Description: Based on review, the provider failed to ensure that children's records are maintained with the required information. Evidence: Reviewed 5 children's records and found that CH1, CH2, CH3 and CH4 did not have documentation of current, annually updated emergency contact information.

Plan of Correction: Am currently updating

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that the family day home is maintained in a clean, safe and operable condition. Evidence: Observed that there was a hall behind the door in the playroom bathroom and a second hole along the wall under the posted documentation.

Plan of Correction: The wholes have been covered

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface was nonabsorbent. Evidence: Observed that there was a small hole on the diaper changing pad near the cribs in the rear playroom. There was a second mat with a large hole, but staff insisted that it is not being used.
